The Development of a Patient Navigator Program for Seniors with Complex Needs in Toronto, ON

    This poster is great illustration of the power of qualitative methods to measure the impact of a QI intervention. Complex care of older adults is a relevant and important topic in healthcare today and the outcomes are most certainly nuanced and multi-dimensional. Capturing the perspective of the patient and their caregivers is important work. The two themes really resonated and align well with the aim of the intervention. I look forward to hearing more about this work in the future including the addition of cost effectiveness data and health outcomes. What's next for this project? Will the pilots continue or expand?
